New Patient Registration

To register with our practice you will need to live within the practice boundary, which due to the rapid expansion of housing developments within the Pontardulais area is reviewed regularly to ensure that we can safely deal with the number of patients on our list.

You must then complete an online registration form below or request one from the main reception desk.

If you or any of your family registering are on any regular medication then please bring the prescription slips from your previous GP or a medication printout to provide evidence of this.

We are not able to start prescribing regular medicines without evidence of your previous history and this is not always sent to us electronically.

Patient registration requests will only be accepted from patients who are living within the area indicated in the map below!

We are currently experiencing high volumes of new registrations due to the many building developments in the area, and may need to review these boundaries again in the future.

If you move house it is vitally important that we have correct contact details. Once you move outside of the practice boundary you will be asked to register elsewhere. Exceptions will only be made for patients who are receiving ongoing treatment for a serious condition – and then only at the discretion of the GP involved in their care, and only if they are still living within a reasonable distance of the surgery. All other patients will be expected to register with another GP as soon as they move home.

Change Your Contact Details

Temporary Services

If you are away from home and need to see a GP in an emergency then you can register as a temporary patient. You will still be registered at your existing surgery and will not need to re-register when you return to your home address.

If you are requesting repeat medication then you will need to provide proof of medication (either a repeat prescription slip, printout obtained from the surgery where you are registered or medication boxes that you currently have).

To register as a temporary patient simply contact the practice you wish to use. You cannot register as a temporary patient at a practice in a town or area where you are already registered.
